• Initial Intake Appointment (60-90 min) - Includes complete health history/intake form, Dietary Goals/Concerns, and review over your food journal.
• 3-5 day Food Journal is required prior to our initial intake visit.  We can discuss this upon our initial 10-15 min preliminary phone call.  This form can be emailed over to you.
• 3 Follow Up Appointments - Including the items listed below
• Ongoing diet journal analysis
• Individualized meal plan & recipes
• Possible herbal/vitamin supplementation guidance
• Follow up emails including diet/lifestyle/recipes/supplementation recommendations
• Ongoing email and phone support

Individual Nutritional Coaching Session - $75 for 1 Hr

• Basic analysis of your food journal
• 3-5 day food journal prior to visit is required. - Food Journal form can be emailed to you directly prior to staring your journal.
